Muslim Funeral Services representative picks up the deceased body from Coroner’s office/ Hospitals/Retirement home/Airports or the Home and brings it to the Funeral Services Home..
The family/guardian of the deceased need to perform the following tasks:
Provide two government-issued photo ID’s of the deceased (Acceptable ID’s are Drivers License, Passport or Citizenship card)
Fill out three forms, which are Statement of Death – Form 15, Family Questionnaire & Burial Services & Cemetery Information (Family to provide grave plot number and buyers name on this form if they already have purchased a plot.)
BFS’s members contributing pay for the Mosque funeral and burial services.
During this process, Mosque Volunteer will assist family in filling out forms and documents
Verify all information including review the “Death Certificate – Form 16”
Arranging Fax the “Burial Services & Cemetery Information” to the Cemetery requesting to open a grave.
Organizing perform the final rites of the deceased according to Islamic rulings, which are limited to body washing and wrapping in a shroud only with the help of volunteers and family members.
Submit “Statement of Death – Form 15“, “Death Certificate – Form 16” along with blank “Burial Permit” to the City Office. The city will issue a “Burial Permit” while keeping other forms in their records.
Organize and perform Janaza prayer at the Mosque
Transport the coffin to the cemetery for burial. (Cemetery accepts burials 7 days a week until 3:30 pm).
Provide 10 signed copies of “Proof-of-Death Certificate” to the family (which may be used to inform Government offices, banks and other institutions)
Take the Statement of Death Form 15, Death certificate – Form 16 (provided by Masjid) and a blank copy of the Burial Permit to the City or Police station (after hours). They will keep the forms and return the Burial certificate after stamping for the family to keep.
Dial 911 for emergency assistance (an ambulance, the fire department or the police) and a coroner (Investigation Officer) will come to your home and will sign a death certificate which gives you permission to contact your BFS Area Coordinator in your location to make arrangements to transfer deceased person to mosque.
If permission is given, call BFS Area Coordinator and Coordinator will contact to the nearest partnering mosque, and will inform them of his/her death. BFS Area Coordinator will then make burial arrangements with the help of the family member or relative. If an autopsy is required, deceased person will be taken to the hospital for autopsy. After the autopsy has taken place and the doctor has signed the death certificate, BFS Area Coordinator Will make arrangement with concern mosque to start deceased person’s funeral process.

Mosque has a well-equipped mortuary to perform washing ghusl (final bath) and prepare the body for burial.
Mosque representative will accompany the next of kin/family member to the hospital and fulfill all the documents for the release of the body. Thereafter, the deceased body will be brought in to the Centre for ghusl (final bath), preparation, and Salatul Janazah. It will then be transported to the cemetery for burial.
Mosque will issue a “Proof of Death Certificate” (Temporary) to the family of deceased person. This certificate may be required for the settlement of legal issues such as bank accounts, insurance claims, inheritance, etc. The next of Kin is required to apply for a legal “Death Certificate” via provincial service authority.
Please ensure that the information of the deceased on the “Death Certificate” is accurate. If you notice anything that is incorrect, please request the authorities to correct it. Contact BFS 24X7 contact our Area Coordinator. Provide your contact info, time of death and place of death. Pick up will be conducted by the nearest partner mosque. Therefore pick up time will be maintained by the mosque.
